2012 Patient and Family Education Calendar
Please find the 2012 Patient Education Calendar. This year’s theme is “The SEARCH is On.” Each month has an interactive word search game for dialysis patients and family members to play. In December 2011, Southeastern Kidney Council mailed copies to every dialysis center in the area.
This calendar was developed by the 2011 Southeastern Kidney Council Consumer Committee as a tool to educate all patients and family members on various topics.
Each month, we send materials related to the month’s topic to utilize in your patient and facility education. Below, please find copies of materials that were sent to your center:

- What is the Dialysis Performance Facility Certificate Poster: Please display in the patient and family waiting room and attach your 2 page Dialysis Performance Facility Certificate to the poster. You can access your Dialysis Performance Facility Certificate from www.dialysisreports.org.
- Quality Incentive Program: A Guide for the Dialysis Community Brochure: Please place copies in the patient and family waiting room.
