Mission Statement:

To improve the lives of people with or at risk for End Stage Renal Disease by promoting and advancing quality of care.

Goals and Expectations

Network 6 Commitment and Expectations for Achieving the National Goals.

Assessing and improving ESRD care is a complex process. This process requires partnership with multiple entities to include CMS as payer of services, providers of care, facility staff and consumers. The Network serves as a liaison between these groups, providing data, education and assistance to patients and providers to assist them to improve care. This cooperation is essential to reach CMS' goals.

The Network commits to:

  • Collect patient-and facility-specific data related to improving outcomes of care with reasonable submission deadlines
  • Analyze and report information to help ESRD providers and health professionals improve the treatment and control of renal failure in the Network
  • Develop, conduct, and evaluate Quality Improvement Projects and special studies targeting specific areas of patient care
  • Provide education, technical assistance, and tools to help patients and facilities to improve the quality of care

The Network expects facilities to:

  • Respond to all requests from the Network for patient demographic and outcomes data in a timely manner
  • Incorporate these reports into facility Quality Improvement programs to identify areas for improvement
  • Cooperate with all CMS-approved Network activities
  • Respond promptly to any facility-specific problems or concerns identified by the Network
  • Attend Network workshops and symposia
  • Publicize patient workshops and encourage patient participation
  • Use the educational materials and tools provided to facilities by the Network
  • Contact the Network for Quality Improvement/Assessment assistance
