Mission Statement:
To improve the lives of people with or at risk for End Stage Renal Disease by promoting and advancing quality of care.
Data Awards
Each year, the Southeastern Kidney Council recognizes Network 6 area dialysis facilities that have achieved 100% compliance on accuracy and timeliness of forms submission to the Network office. The following criterion describes the required elements needed to achieve 100% compliance:
Medical Evidence Report (CMS 2728)
- Timeliness is defined as 45 days from the date the patient is determined to be End Stage and has started regular dialysis at the facility, had a transplant for greater than 36 months, or the patient has recovered function and has been off dialysis for greater than 12 months
- Accuracy is defined as all fields on the form are complete and accurate
Death Notification (CMS 2746)
- Timeliness is defined as 30 days from the date of death
- Accuracy is defined as all fields on the form are complete and accurate
