Improving Care
The Southeastern Kidney Council is dedicated to improving quality of care in ESRD facilities.
The following indicator goals were set by the Medical Review Board:
- Adequacy Best Practices
- Adequacy Toolkit of Educational Information
Adequacy of Hemodialysis
90% of Network 6 facilities will reach the CMS goal of 80% of patients with URR > 65%.
Anemia Management
95% of facilities will reach the CMS goal of 75% of patients with HgB > 11 g/dL.
Vascular Access
- By March 2009, at least 45.8% of all prevalent hemodialysis patients (adults >= 18) should use an AV fistula with a long-term goal of 66%.
- 100% of patients with an AV Graft will have stenosis monitoring.
- Less than 10% of all prevalent hemodialysis patients (adults >= 18) should be maintained on catheters >= 90 days with no internal access.

Immunization (inclusive of Hepatitis B, Pneumococcal, and Influenza)
90% of patients will be immunized.
Quality Assessment and Perfromance Improvement (QAPI)
QAPI (MAC) Toolkits: MAC Toolkit - The Medical Advisory Council (MAC) of the Forum of ESRD Networks has developed four toolkits to assist facilities in meeting the Conditions for Coverage.
